The photograph is of Richard Fawcett. He was married to Grace, the daughter of John Waddington, himself a prominent 19th century colliery manager in the Burnley and Padiham area. Mr Fawcett became the manager at Hapton Valley colliery. Mr Fawcett was killed, along with a Thomas Wood, while they were ascending an air shaft.
The accident occurred on 26th February 1889 and is reported in the local newspapers on the 27th February and the 2nd March 1889.
It would appear that on being raised in the shaft, they were a few yards from the surface, when the rope broke causing them to fall down the shaft and were killed instantly.
